Optimal Healthcare Ltd has launched Primary Care Unlimited, a membership-based primary care programme designed to give residents of the Cayman Islands open-ended access to GP-led healthcare for less than $2 per day.
The programme addresses a well-documented gap in the islands' healthcare landscape. While the Standard Health Insurance Law ensures residents have access to basic hospital and specialist care, primary care coverage under most insurance plans is limited financially to one or two GP visits per year. For the majority of residents, sustained access to a trusted GP for prevention, screening, chronic disease management, and
wellness has been financially out of reach.
Primary Care Unlimited changes that.
What the Programme Delivers
Members pay a fixed monthly fee starting at $60 KYD and receive:
- Unlimited primary care consultations, free at the point of care
- A comprehensive annual health and wellness medical, assessing a minimum of 145 biomarkers across laboratory screening, body composition, cardiovascular function, and clinical assessment
- Concierge-level care coordination from a dedicated clinical team
- A whole-person health assessment built around eight dimensions of health: preventive medicine, risk reduction, physical activity, nutrition and build, sleep, stress, mental health, and social wellbeing
The programme operates alongside health insurance, not as a replacement. Where applicable, claims are submitted to insurers on the member's behalf. Any co-pay that arises is taken from the membership fee pool. The member pays nothing at the point of care, and care is not limited by a benefits schedule or finite access.
Why It Matters
The Cayman Islands faces significant and growing challenges from non-communicable diseases. Diabetes, cardiovascular disease, and obesity are conditions that develop gradually, are detectable early through screening, and respond to sustained clinical support. However, sustained support requires consistent access to
a primary care relationship. When that access is constrained by insurance limitations or cost, prevention does not happen and conditions progress unchecked.
"The gap in our healthcare system is not acute access for emergencies or hospital care," said Andrew Vincent, Managing Director of Optimal Healthcare. "Even basic insurance provides reasonable cover when hospital care or specialists are needed. The gap is the everyday relationship with a GP who knows you, who screens you
proactively, who manages your health trajectory over time. That is what most people in the Cayman Islands do not have. Primary Care Unlimited is built to fill exactly that gap."
The Annual Medical
Every member receives a comprehensive annual medical that goes well beyond a standard check-up. The investigation panel at the Comprehensive tier (Comprehensive is in fact the lowest level) includes 80 laboratory biomarkers and 65 non-laboratory biomarkers. At the Enhanced tier, this expands to 96 and 84 respectively. At Cayman Standard Fees, the investigation panel alone is worth more than twice the entire annual membership fee - before a single GP consultation.
Three Tiers

All tiers include unlimited consultations, the annual medical, and concierge coordination. Higher tiers expand the diagnostic panel and add health coaching and additional body composition assessments.
The GP Team
Members choose their own GP from a team of lifestyle-medicine focused practitioners:

- Dr Rebecca Deasey - General Practice
Clinically, the practice is led by Dr Sara Watkin, a tertiary-trained Consultant Paediatrician and Neonatologist with over 30 years of clinical experience, and importantly, a background in healthcare governance and physician quality management and revalidation, in her former role as Associate Responsible Office for the world-renowned University College London Hospitals NHS Foundation Trust.

Primary Care Unlimited is also available as an employee benefit.
